Use this as a quick reference while filling in the printed form.

| Ship From / Ship To | The physical pickup address and the physical receiving address. |
| SID# / CID# | Optional reference numbers on the shipper side and the receiving side. |
| Location No. | The door or bay number at a large facility, if you have it. |
| Third Party Freight Charges - Bill To | Only used when a third party is paying the freight charges. |
| Bill of Lading No. | Your own number for this document. It is not a tracking number. |
| Carrier Name / SCAC | The carrier and its SCAC carrier code. |
| Pro No. | Assigned by the carrier. Leave it blank until you have it. |
| Trailer No. / Seal Number(s) | Record the seal number exactly as printed. |
| Freight Charge Terms | Mark one of Prepaid, Collect or 3rd Party. |
| # Pkgs., Handling Unit QTY and Type | How many pieces, and what they are packed in. |
| Commodity Description | Plain English and specific. |
| Weight | The weight of that line of goods. |
| H.M. (X) | Mark it for hazardous materials when applicable. |
| NMFC No. / Class | Freight classification fields for the commodity line. |
| COD Amount / Fee Terms | Used when Collect on Delivery applies. |
| Signatures | Shipper and carrier signature areas, Pickup Date, and who loaded the trailer and counted the freight (Shipper or Driver). |
